Self Referral

It is increasingly possible for patients to self-refer directly into other services to receive treatment. This helps to reduce the demand for GP appointments and is more convenient for the patient.

If you think you need treatment for any of the following problems, you will not normally need to see a doctor first. Please contact the provider directly for further information and to book an appointment.

Adult ADHD Referrals

Current Situation & Your Options

Due to very high demand, the NHS ADHD service in Derbyshire is temporarily closed. The good news is that you can still access an assessment through a process called Right to Choose. This allows you to choose a private provider, and the NHS will fund the assessment.

Choosing a Provider

Before making your decision, it’s important to check the following:

  • You meet the provider’s referral criteria
  • They are able to prescribe medication (if that’s something you may want)
  • You’re aware of their waiting times

We understand you may want to be seen quickly, but it’s essential to choose the provider that best suits your needs – not just the one with the shortest wait.

Next Steps – How to Request a Referral

To start the referral process, please complete all five of the documents listed below and return them to us:

  1. ADHD Screening Questionnaire
  1. A written or typed statement detailing:
    • Your symptoms and concerns – if you have not had these since childhood, your referral is not likely to be accepted
    • How ADHD affects your everyday life (home, work, relationships)
    • What you’ve tried to manage these symptoms
    • What you hope to gain from a diagnosis
      (Please provide as much detail as possible to help your referral succeed.)
  1. The name of the Right to Choose provider you’ve selected
  1. The referral proforma downloaded from the provider’s website(Note: Your GP cannot complete this form for you.)
  1. The ADHD Right to Choose Disclaimer Document, signed by you

    Please return the documents to us by bringing them to the surgery. Alternatively you can post/email these to us.

    We’ll begin your referral only when we receive all five items. Incomplete submissions may cause delays or be rejected.

    Reminders & Timeframes

    If any documents are missing, we’ll send you reminders. We’ll keep your referral open for 21 days to give you time to send everything. If we don’t hear back, we’ll assume you no longer wish to continue and will close your referral – this helps our admin team support other patients too.

    No Need to Speak to a GP First

    There’s no requirement to speak with a GP before submitting your documents. We’ll review everything you send in to assess eligibility.

    After You’re Referred

    Once your referral is sent to your chosen provider, you’ll hear directly from them. From that point on, any questions or updates should go directly to your provider – our surgery will no longer be involved in the process.

    Physiotherapy

    This Out-patient Physiotherapy Service provides assessment, diagnosis and treatment for a wide variety of musculoskeletal problems (i.e. injury, damage or disorders of the joints and other tissues e.g. bones, ligaments, tendons, muscles, nerves). It also offers advice and education on the self-management and prevention of musculoskeletal problems.

    The service is provided by Derbyshire Community Health Services at Dronfield Health Centre and at Eckington Health Centre

    Women’s & Men’s Health Physiotherapy

    This Out-patient Physiotherapy Service provides assessment, diagnosis & treatment for:

    • Pelvic floor problems (bladder or bowel urgency or leakage)
    • Pelvic or genital pain
    • Pain with sexual activity
    • Pelvic organ prolapse
    • Prenatal or postnatal muscle, nerve or joint pain

    The service is provided at Chesterfield Royal Hospital.

    Neurological Physiotherapy

    This Out-patient Physiotherapy Service provides advice, education, exercise, equipment or onward referral for patients who have a neurological condition diagnosed by a neurological consultant. Conditions include:

    • Multiple Sclerosis (MS)
    • Parkinson’s
    • Previous stroke
    • Cerebral Palsy
    • Peripheral Neuropathy

    This service is provided at Chesterfield Royal Hospital.

    Mental Health & Talking Therapies

    This therapy service is provided for patients over 16 years of age with the following problems:

    • Anxiety
    • Low mood
    • Depression
    • Stress
    • Panic
    • Anger
    • Trauma
    • Bereavement
    • Relationship difficulties
    • Phobias

    Everyturn

    The service is provided by Everyturn (previously known as Insight Healthcare) for patients age 16 years and over.

    The Service is provided by Trent PTS for patients age 16 years and over at West Bank House, Albion Road, Chesterfield, S40 1LL.

    Confidential service for people in despair and who feel suicidal. For all ages.

    An online counselling and emotional well-being platform for adults, accessible through mobile, tablet and desktop and free at the point of use.

    Relate

    Counselling service (this service is not free to use – minimum cost is £30). For patients age 5 to 18 years.

    This service is called Safe Speak and is provided by Relate.

    Podiatry (Foot care)

    The Community Podiatry Service provides assessment, diagnosis and treatment to a range of patients with lower limb and foot problems, including:

    • People with long term conditions such as diabetes, vascular disease, amputees, connective tissue disorders, stroke, Parkinson’s disease
    • People with multiple and complex needs e.g. dementia, falls
    • People with biomechanical problems e.g. gait /postural problems
    • People requiring nail surgery
    • People with severe foot and ankle problems and conditions

    The service is provided by Derbyshire Community Health Services at Dronfield Health Centre and at Eckington Health Centre.

    Audiology (Hearing loss)

    The service is provided to patients over 55 years of age and struggling with hearing. Patients can use any of these providers by calling them directly.

    Drug & Alcohol

    Derbyshire Recovery Partnership provides services for people in Derbyshire with a drug and/or alcohol problems. The teams offer different levels of support from brief advice and harm reduction to intensive structured 1-to-1 and group work.

    Derbyshire Recovery Partnership also manages all substance misuse substitute prescribing for drug or alcohol treatment across Derbyshire.

    The service is for any adult (18+) who wishes to address any issues they have caused by the use of drugs or alcohol. This can include binge drinking, the use of New Psychoactive Substances, as well as physical alcohol or drug dependence.

    Derbyshire Recovery Partnership (DRP) is a drug and alcohol treatment service between Derbyshire Healthcare NHS Foundation Trust, Derbyshire Alcohol Advice Service (DAAS), Phoenix Futures and Intuitive Thinking Skills.

    Stop Smoking

    This service is for anyone over 12 years old & provides support, help and advice for you to successfully stop smoking for good. The fully trained Stop Smoking Advisers can support you through your quit attempt with tips, advice and stop smoking products or medication on prescription.

    The Service is provided by Live Life Better Derbyshire at various locations across Derbyshire, including Moss Valley Medical Practice.

    Weight Management

    This service is for anyone over 16 years old & with a Body Mass Index (BMI) over 25.

    The service supports you to set and achieve your own weight loss targets and to maintain this weight loss long term. It is different to other weight loss programmes as it focuses on what’s happening in both the body and the mind to help you achieve long-term weight loss and wellbeing.

    This service provides a helpline & live chat, counselling & coaching and one day courses for adults over age 18 affected by an Autism Spectrum Condition (ASC). The service is also available for carers of adults living with an ASC. The service is available for residents of Derbyshire only.
